A School seeking designation as an Achievement District School, pursuant to A.R.S. 15-2141, shall file an Application with the Board, which may be modified from time to time by the Board.
Applications failing to comply with the requirements of A.R.S. 15-2141 shall be denied without prejudice to the subsequent filing by such School of an application complying with such requirements.
The accuracy and completeness of the information contained in an Application shall be vertified by the Chief Executive Officer or Chief Financial Officer of the School.
